Contract of Agreement for Alcohol or Other Drug Offense (revised June 2010) - to be completed for every student in violation of school discipline policy 4-3(b), #9 and copy faxed to School Security Coordinator (704-282-2171) at time of infraction. Form can also be found on the main school web site under Administrative Forms.
